A lead professional should be appointed when a wellbeing need has been identified and can only be met by a targeted intervention*

The Health Visitor is the named person throughout the Pregnancy until the child starts Primary School*

Social Workers always take on the role of lead professional if they are involved*

The lead professional is responsible for all practitioners and services they are co-ordinating*

National GIRFEC Practice Model questions

There are five questions that are asked when considering the Wellbeing Indicators*

There is an Interactive Triangle that young people can use to help share their views*

The Resilience Matrix should be used in every assessment*

The wellbeing indicators are used to structure the Child/Young Person's Plan ('My Plan') and improve their outcomes*

A child/young person can have a separate assessment completed by each agency supporting them*

An assessment should be completed proportionately depending on the needs identified for the child/young person when using the GIRFEC Practice Model*

A chronology is a journal of thoughts and feelings by the child/young person*

A chronology is evidence based and kept in date order*
